Central and Eastern Sydney PHN is pleased to announce the launch of GP Connect on 8 December 2025. This new service ensures that residents entering Residential Aged Care Homes (RACHs) without a usual GP can access timely, high-quality primary care during their transition.
What is GP Connect?
GP Connect provides short-term general practice care (up to 8 weeks) for residents who do not have an existing GP relationship. During this period, the service will work to source and match residents to a regular GP, ensuring continuity of care beyond the initial support phase.
The service works in close collaboration with a Nurse Practitioner, who triages and manages incoming requests, conducts onsite visits when required, and offers remote clinical support. At the conclusion of the care period, GP Connect will provide a comprehensive clinical handover summary to the RACH and facilitate transition to a long-term GP, ensuring continuity of care.

This a free service for residents and Residential Aged Care Homes in the CESPHN region who can refer residents who do not have a regular GP to the service.

Please note:
GP Connect provides standard GP cover for medically stable residents only. This service is designed to complement existing Hospital Outreach and Acute Care teams, not replace them.
