Healthy Ageing Hubs

CESPHN has recently commissioned two neighbourhood centres in our region to establish Healthy Ageing Hubs to support older people to live well and independently in the community.

The Healthy Ageing Hubs are available in two locations in our region, in the Inner West, at Newtown Neighbourhood Centre, and in the Sutherland Shire, at Gymea Community Aid and Information Service.

About the Service

The Hub provides practical support, service navigation assistance and guidance to older people living in the community. The Hub aims to help people maintain independence and social connection, and to empower people to look after their health and wellbeing as they age. It also offers a pathway for health care providers to refer their patients who might be socially isolated, in need of support and guidance on a range of matters or require assistance accessing aged care and other services.

Target Population

Both the Inner West and Sutherland Shire Hubs are available to serve older people, their carers and family members who live in the Central and Eastern Sydney PHN region.

The Healthy Ageing Hub will also serve the local community, through a series of talks and presentations delivered to a range of audiences, aimed at promoting healthy ageing and dispelling myths about older age.

Referral Process

The Hub is designed to support primary health care providers by offering a pathway to social support and care services for their patients.

A Healthy Ageing Advisor is available at the Hub to offer support either face-to-face or over the phone. The Hub offers both a drop-in service as well as appointments which can be booked in-person at the Hub, over the phone, or online.

A referral is not required to see the Healthy Ageing Advisor, although health care providers and community service organisations can refer older people to the Hub.

Assistance Provided

The Healthy Ageing Advisor will provide guidance and practical assistance to older people to help them live independently and support health and wellbeing, based on individual needs. This may include guidance on how to access My Aged Care, advice on how to access local services and programs, and planning for older age.

The Healthy Ageing Advisor will connect people to local services and programs as required. This may include health, social, legal or housing and accommodation services, financial, social or mental health support, group exercise and social programs, or care finder services for people requiring specialist and intensive support with My Aged Care.

The Healthy Ageing Hub will also engage the Inner West and Sutherland Shire communities to raise awareness of healthy ageing via a series of presentations and talks on a range of topics designed to dispel myths about ageing and empower people to live well as they age.
