An Annual Health Assessment (AHA) is a comprehensive and structured assessment of a patient. Each year, individuals with intellectual disability are eligible for a health assessment covered by Medicare. This assessment is crucial for understanding their health needs and ensuring appropriate follow-up care.
The Comprehensive Health Assessment Program (CHAP) is an evidence-based tool for conducting these assessments and is endorsed by the Australian Government.

HealthPathways: Health Assessment for People with an Intellectual Disability
The CHAP assists with gathering important insights into an individual’s current health, ensures regular screening takes place and allows GPs to gather an individuals health baseline.
The CHAP consists of two parts and an action plan.
Part 1 is filled in by the individual and the people who know them best and can speak to their health over the past year.
Part 2 is completed by the Practice Nurse and individual’s regular GP.
Action Plan is filled in by GP in collaboration with the individual and/or their caregivers. A copy of the Action Plan is to be given to the individual and/or caregivers and another kept with the persons record.
Project GROW’s Annual Health Assessment (AHA) checklist is designed to assist service providers in preparing an individual and their support team to attend an AHA with confidence. The checklist outlines what is required before, during and after the appointment.
Project GROW also provides specialised training for primary care and disability support providers. This training equips professionals to fully understand and effectively execute their role in the assessment process, facilitate clear communication and implement adjustments that support everyone to have a good appointment.
