Chronic Conditions Management (CCM)

Significant changes to the Chronic Conditions Management (CCM) MBS items took effect on 1 July 2025, marking the first major update to the program in 20 years.

The updated arrangements simplify CCM items, making them easier for health care professionals and patients to use. The changes promote continuity of care, encourage regular review of CCM plans, and support communication across a patient’s multidisciplinary care team. They also ensure that patients who were already receiving CCM services continue to access the care they need.

Two‑year transition arrangements are now in place so existing patients do not lose access to services.
