CESPHN is seeking Expressions of Interest from small and solo general practices across the CESPHN region to participate in the Diabetes Care Clinic program delivered by Diabetes Australia.
Practices will benefit from access to diabetes-related allied health support delivered by Diabetes Australia either within their practice or virtually, at no cost for eligible patients.
Financial support to the value of $5,000 (excluding GST) will be offered to participating general practices. The program will run until June 2027.
