MyMedicare – is your practice registered?

MyMedicare is a voluntary patient registration initiative that strengthens the ongoing relationship between patients, their general practice and preferred GP.

It supports improved continuity, coordination and quality of care, particularly for people with chronic and complex conditions, by enabling practices to better understand and plan care for their regular patient population.

MyMedicare underpins several key primary care initiatives, including Chronic Condition Management (CCM), the General Practice Aged Care Incentive (GPACI), the Bulk Billing PIP, and Better Access changes. Together, these initiatives support proactive, team‑based care and recognise the value of ongoing doctor–patient relationships rather than single visits. They also assist practices to deliver care that is coordinated, patient‑centred and aligned with broader primary care reform.

By identifying their regular patients, practices are better positioned to plan services, support multidisciplinary care teams and engage with reform in a way that is practical, sustainable and manageable alongside day‑to‑day operations.

CESPHN continues to support practices to understand MyMedicare and implement related initiatives in ways that are clear, workable and aligned with everyday workflows.
