POLAR Transition Completed – PEN CS Licences Closure & Next Steps for Your Practice

Central and Eastern Sydney Primary Health Network (CESPHN) is pleased to advise that the transition from PEN CS to POLAR has now been fully completed across our region.

As part of this change, PHN-funded PEN CS CAT4 licenses have ceased, and POLAR is now the primary population health intelligence tool supported by CESPHN.

Thank you to all practices for your cooperation and support throughout this transition.

With POLAR now in place, practices can benefit from improved:

  • Data visualisation and reporting
  • Clinical insights to support quality improvement
  • Population health planning
  • Performance monitoring and PIP QI activities

If your practice requires training or support with POLAR, please contact our team. We remain deeply committed to helping practices make effective use of the tool.

Thank you for your collaboration throughout this transition. We look forward to continuing to support your quality improvement activities using POLAR.

For any questions, please contact our Digital Health Team at digitalhealth@cesphn.com.au
