Quality Improvement (QI) Delivery

Improving Health Outcomes for People with Intellectual disability

The Primary Care Intellectual Disability Inclusion Program (PCIDIP) supports general practices in the Central and Eastern Sydney PHN (CESPHN) region to build their capacity to care for people with intellectual disability and assist their families and carers. 

The program offers financial support of $1500 to general practices to complete two Quality Improvement Activities (QIAs).  

To successfully complete the Intellectual Disability Primary Care Inclusion Program, participating practices must: 

  1. Complete the following two Quality Improvement activities: 
  • Data cleanse and code patients with intellectual disability in clinical software. 
  • Annual Health Assessment (AHA) audit and development of a recall and reminder process for patients with intellectual disability that have not had an AHA in the last 12 months.   
  1. Report deidentified data from the Quality Improvement activities  
  1. Ensure all participating staff complete a program evaluation survey.

Participants can self-report Continuing Professional Development (CPD) hours for these activities through relevant bodies like the RACGP.  

Each QIA is estimated to take between 20 minutes and one hour to complete. One key staff member with POLAR access is required to coordinate the activities and support relevant GPs to review patient files.

The below resources are designed to support QIA completion: 

  • Project GROW resources: Including Easy English tools, multilingual guides, and CHAP‑aligned checklists developed by CESPHN to help people with intellectual disability, their supporters, and GPs prepare for and complete Annual Health Assessments. 

On request, CESPHN can deliver short, targeted education sessions for your whole practice team: 

  • Intellectual Disability Health Care  
  • Annual Health Assessments & Reasonable Adjustments  

These sessions are optional and designed to support (not replace) the QIA requirements.
