About the Diabetes Care Clinic Expression of Interest (EOI)

CESPHN is seeking Expressions of Interest from eligible small and solo (1-5 GP FTE) general practices to participate in the Diabetes Care Clinic Program, supporting patients with prediabetes and type 2 diabetes.

General practices selected to participate will receive $5,000 (excluding GST) in funding support. The program will run until June 2027.

While the Expression of Interest (EOI) is currently open to practices within the Inner West, we are pleased to share information about the initiative with all practices across the region to support awareness and future opportunities.
