HealthLink Smartforms for both continuation referrals and new patient referrals to SLHD services

HealthLink Electronic referrals (eReferrals) are now the preferred method for over 95 outpatient services across Sydney Local Health District (SLHD).

Did you know that HealthLink Smartforms can be used to send:

  1. New patient referralsfor patients that have not been seen by the service, as well as,
  2. Continuation referrals for ongoing specialist care and annual follow up appointments to Sydney LHD services

This is reflected in the Referral type section of the form – a quick and easy way to provide an updated referral for a patient requiring ongoing specialist services. Please use HealthLink

SmartForms when making your 12 month continuation of service referrals, as well as initial or new referrals.

New or Continuation referrals sent under the wrong referral type cannot be appropriately billed and may be rejected and returned to the referrer for a new referral to be lodged.

Requests for Bone Mineral Density (BMD) studies and Respiratory Function testing should always be identified as New referrals.
