This is reflected in the Referral type section of the form – a quick and easy way to provide an updated referral for a patient requiring ongoing specialist services. Please use HealthLink
SmartForms when making your 12 month continuation of service referrals, as well as initial or new referrals.
New or Continuation referrals sent under the wrong referral type cannot be appropriately billed and may be rejected and returned to the referrer for a new referral to be lodged.
Requests for Bone Mineral Density (BMD) studies and Respiratory Function testing should always be identified as New referrals.
