The Complex Care Service

Did you know there is a medical service delivered by the Centre for Disability Studies for people with intellectual disability?

The Complex Care Service provides comprehensive healthcare assessments, healthcare plans, advice and referral navigation for people with intellectual disability aged 15 years and above who have an unmet health need. The service operates 3 days per week (Wednesday to Friday). It provides in person appointments at the Bethany Centre in Strathfield and offers telehealth appointments across NSW.

The service can also offer healthcare clinics in the community hosted by service providers in settings such as supported accommodation or day programs.  This initiative allows multiple patients to be seen in one setting, reduces the travel required for patients and enables the appointment to occur in a familiar, less stressful environment.

The Complex Care Service is open for  referrals

Classification Questions from 2021-2023

Q. We are interested in joining the program for multiple locations within the CESPHN. We would like to know- if there is a minimum number of practice staff, GP or nurses that is required for those training sessions and QI activities?

A. No, there is no minimum participation requirement. The program is open to the entire practice, and we encourage all relevant staff, including GPs, nurses, administration staff and allied health clinicians to participate.

If multiple practices are applying, each practice must submit a separate EOI to be eligible for the $5,000 funding per site. If you prefer to combine the program across locations, only one EOI is required, but funding will be limited to a single payment of $5,000.

Q. Does the practice require the presence of a practice nurse to undertake the Intellectual Disability Primary Care Inclusion Program.

A. No, a practice nurse is not required. The practice only needs the capacity to undertake training and complete Quality Improvement activities.
