Improving access to GP care for RACH residents
CESPHN is pleased to announce the launch of a new initiative, GP Connect, commencing on Monday 8 December 2025. This initiative is designed to support Residential Aged Care Homes (RACHs) and their residents in accessing timely and consistent general practitioner (GP) care.
GP Connect offers short-term GP services for a period of up to 8 weeks, providing immediate medical support to residents who may not currently have a regular GP. During this time, our team will work closely with residents, their families, and care home staff to identify and match each resident with a suitable long-term GP, ensuring continuity of care beyond the initial period.

Please note:
GP Connect provides standard GP cover for medically stable residents only. This service is designed to complement existing Hospital Outreach and Acute Care teams, not replace them.
