Improving access to GP care for RACH residents

CESPHN is pleased to announce the launch of a new initiative, GP Connect, commencing on Monday 8 December 2025. This initiative is designed to support Residential Aged Care Homes (RACHs) and their residents in accessing timely and consistent general practitioner (GP) care.

GP Connect offers short-term GP services for a period of up to 8 weeks, providing immediate medical support to residents who may not currently have a regular GP. During this time, our team will work closely with residents, their families, and care home staff to identify and match each resident with a suitable long-term GP, ensuring continuity of care beyond the initial period.

This service aims to:

• Improve GP access for new residents without a GP.
• Reduce delays in medical attention and improve health outcomes.
• Support RACH staff in managing residents’ health needs more effectively.
• Facilitate smooth transition from hospital to RACH for patients unable to return home.
• Facilitate a smooth transition to long-term GP care.

Please note:

GP Connect provides standard GP cover for medically stable residents only. This service is designed to complement existing Hospital Outreach and Acute Care teams, not replace them.

GP Expression of Interest

We are actively seeking local GPs who are interested in taking on new RACH residents and willing to be contacted for potential long-term matching of new residents. Your involvement can make a meaningful difference in the lives of older adults in our community by ensuring they receive consistent, high-quality care.
