Social Rx®
Commonwealth Psychosocial Support (CPS) Program
Primary & Community Care Services (PCCS) is providing the Social Rx ® program in the CESPHN region as part of the Commonwealth Psychosocial Support (CPS) Program.
The CPS program provides psychosocial support services to assist people with severe mental illness and reduced psychosocial functional capacity who are not receiving psychosocial supports through the National Disability Insurance Scheme (NDIS).
Program Overview
Social Rx® uses a combination of individual and group-based activities, and works within a person’s context (i.e., family, friends, peers, local community) to assist them to create their own vision, discover their needs, identify and utilise existing resources and assets, and develop their skills and capabilities to realise their potential.
Through Social Rx®, link workers reach into the community to identify individuals that could benefit from a social prescribing approach to enhanced health and wellbeing. They co-develop action plans and support individuals to achieve their goals and map existing assets within the community. Depending on an individual’s goals, they may then go on to participate in WellTogether® and Skillness® groups to increase their sense of self determination, confidence, and connection.
WellTogether® groups provide supported social participation to build wellbeing, inclusion, and resilience. Activities can be tailored to an individual’s preferences and may include art, photography, bush walking, forest therapy, surfing, yoga, or gardening.
Skillness® groups offer a fun and dynamic pre-vocational program that supports individuals to explore their strengths and passions, gain practical, real-world skills and access work, study, and personal endeavours with a greater sense of empowerment and self-determination.
Eligibility
Social Rx® group programs are available to people who:
- Experience severe mental illness (diagnosis not required);
- Have an associated level of reduced psychosocial functional capacity;
- Live, work, or study in the CESPHN region;
- Are not assisted by the NDIS; and
- Are not receiving any other psychosocial support services that duplicate those provided through CPSP.
