CESPHN is seeking Expressions of Interest from eligible small and solo (1-5 GP FTE) general practices located in Sydney’s Inner West to participate in the Diabetes Care Clinic Program, supporting patients with prediabetes and type 2 diabetes.
Participating practices will be connected with a multidisciplinary model of care, providing patients with access to diabetes-related allied health services delivered by Diabetes Australia, either onsite at the practice or virtually, at no cost to patients.
General practices selected to participate will receive $5,000 (excluding GST) in funding support. The program will run through to June 2027.
An online information session will be held via Microsoft Teams on 27 July 2026, 7.00pm to 8.00pm.
