Active8 Physical Health and WorkWell Employment Support
Commonwealth Psychosocial Support (CPS) Program
Neami National is providing the Active8 Health Coaching and WorkWell Employment Support programs in the CESPHN region as part of the Commonwealth Psychosocial Support (CPS) Program.
The CPS program provides psychosocial support services to assist people with severe mental illness and reduced psychosocial functional capacity who are not receiving psychosocial supports through the National Disability Insurance Scheme (NDIS).
Program Overview
Active8 Health Coaching
Coaching for Physical Health is a program that recognises that positive physical health change is associated with improvements in overall health, wellbeing, and quality of life. Each individual works with a Peer Support Worker trained to empower the consumer over 8-10 sessions to achieve their identified physical health goals.
Eat Plant Learn is a series of 2-hour, non-sequential group workshops across a six-week period with a focus on healthy eating. Based on evidence demonstrating the benefits of activities in nature, gardening and healthy cooking skills, this program adopts an integrated eco-health approach to support consumers to improve their physical and mental wellbeing.
Kick The Habit Tobacco Management is specifically designed to support people living with mental illness to reduce their dependence on tobacco products. It offers 1:1 coaching supporting consumers to develop and implement an individualised smoking management plan based on their personal reasons for reducing or quitting, including Nicotine Reduction Therapy to minimise symptoms associated with nicotine withdrawal and reduce contraindication of mental health symptoms.
WorkWell Employment Support
WorkWell provides a wraparound service to people experiencing mental illness, unstable housing, physical health barriers, and/or low literacy levels. The aim of the program is finding employment opportunities that meet an individual’s interests, needs, and future goals, and to support recovery through the social inclusion that happens in a workplace.
WorkWell consists of:
- Individualised, intensive support
- Rapid job search followed by placement in paid employment
- Time-unlimited in-work support for both the employee and the employer.
Eligibility
The Active8 and WorkWell programs are available to people who:
- Experience severe mental illness (diagnosis not required);
- Have an associated level of reduced psychosocial functional capacity;
- Live, work, or study in the CESPHN region;
- Are not assisted by the NDIS; and
- Are not receiving any other psychosocial support services that duplicate those provided through CPSP.
